
The School Health Office supports the health and safety of our students throughout the school day. Our nurses provide care for illness or injury, assist with managing medical conditions, and help ensure that all required health records and immunizations are current. We work closely with families and staff to promote a safe and healthy learning environment.

Attendance
If your child will be coming in late to school or will be absent, please notify the attendance office at #631-592-3706. You may leave a message on the voice mail at any time. Please state your child’s name and the reason for your child absence.
Emergency Health Contacts
Please ensure that all of your contact information in Infinite Campus is accurate and up to date. In the event of an illness or emergency, it is important that we are able to contact you quickly. All updates and changes regarding phone numbers and email addresses can be completed by going to the parent portal and making the changes needed. Changes for emergency contacts must be done by emailing parentportal@hhh.k12.ny.us and indicating your child’s name and any updated information. Address changes should be requested on the “in district address change” form found on the HHH website under tools-parent.
Illness
We would like to remind all parents to be observant to any signs and symptoms of illness in their children such as fever, vomiting, sore throats, stomachaches, headaches, colds and coughs. Any child exhibiting symptoms of illness should be given ample time at home to recover before returning to school, not only to reduce the spread of infection in the classrooms, but to protect your child from contracting another illness due to his/her weakened state.
**If your child has been running a fever of 100 or higher, please allow a 24-hour recovery period at home after the fever subsides before sending your child back
to school.

Please follow the below guidelines when your child is returning to school after an absence:
Fever: Your child should be fever free for 24 hours without the use of fever reducing medications.
Vomiting/Diarrhea: Your child should be symptom-free for 24 hours and can tolerate eating food without vomiting or diarrhea.
Strep Throat/Pink Eye: It is recommended that your child be on antibiotics for 24 hours and be fever free. Eyes should be free of discharge.
Coughing/Colds: If your child has a persistent cough that is disruptive to his day or has a cold and/or runny nose and generally not feeling well, please give them extra time at home to recover.
Special Medical Needs:
If your child has a medical condition such as a food allergy, asthma, diabetes, a seizure disorder, etc., we strongly encourage you to discuss this with the nurse so that we can help your child adapt to the school day.
Mandated Physicals/Physical forms
All new entrants and students in Pre-K/Kindergarten, 1st, 3rd, and 5th grades are required by New York State to have a current physical examination on file with the school.
**Please note: The only acceptable form for submitting a physical is the official NYS School Health Examination Form. No other form will be accepted.
Vision and Hearing Screenings
Vision and hearing screening will be done by the school nurse on new entrants and students in PK/K, 1st, 3rd and 5th grades. You will be notified if your child does not pass the screenings. Additionally, 5th grade girls will receive a scoliosis screening at school if it was not completed during their physical exam by their physician.
New York State School Immunization Requirements:
All children must be age-appropriately immunized to attend school in New York State. New York State Public Health Law 2164 requires that all students attending PK, and grades K–12 be fully immunized in order to attend school, unless they have a valid medical exemption. Students must receive all required doses of immunizations of the following:
● Diphtheria, Tetanus, and Pertussis (DTaP/Tdap)
● Polio (IPV/OPV)
● Measles, Mumps, and Rubella (MMR)
● Hepatitis B
● Varicella (Chickenpox)
Younger children in Pre-K are also required to receive additional vaccines such as Hib and pneumococcal, depending on their age.
Immunizations are due at registration and no later than the first day of school. Students without immunizations will be excluded from school!
Parents/guardians must:
● Provide proof that their child is up to date on all required vaccinations, or
● Submit a valid medical exemption from vaccination.
**To attend and remain in school, students who are not fully vaccinated or are overdue must receive at least the first dose of all required vaccines and complete all remaining doses in the vaccine series within 14 days of when each dose becomes due. Parents must provide appointment slips for the next vaccines that are due according to the ACIP catch-up schedule in order to remain in progress and continue to attend school.
Important Notice:
Per New York State Department of Health (NYSDOH) regulations, students who are not up to date are subject to exclusion from school. NYS allows 14 days from the first day of school to receive all required immunizations according to the NYS Immunization schedule as long as your child adheres to the ACIP regulations. Please consult with the health office for further information.
Administration of Medication:
Under certain circumstances, when it is necessary for a child to take medication during the school hours, the nurse will collaborate with the family physician and the parents. In order for the nurse to administer medications, including over-the-counter drugs such as Tylenol, eye drops, cortisone cream, etc., we must have written permission from the parents and written medical directions from the physician. The doctor’s note must include the name, dosage and frequency of the medication to be given.
**All medications must be brought to school by an adult in the original labeled container from the pharmacy. All forms can be found at the end of this document.
**At the end of the school year, all medications must be picked up by an adult. Any medication not picked up by the last day of school will be discarded. Medications cannot be stored at school over the summer.
*Please note: New medication orders are required each school year for all grade levels.
**No medications can be sent to school with your child without proper documentation in place. Please do not send any pills, creams, eye drops,
inhalers etc. to school without contacting the nurse first to make sure the mediation can be dispensed properly at school according to state regulations.
Physical Education/Recess Exclusions:
If a child cannot participate in physical education or outdoor recess due to an illness or injury, a parent note for up to one week can be accepted, then a doctor’s note is required. A doctor’s note should be provided if your child is receiving medical care. Please call the school nurse to discuss the nature of your child’s illness/injury in order for any special arrangements to be made. If a child is restricted from physical education, they will also not be permitted to participate in outdoor recess. Students wearing splints and/or casts should not participate in physical education and recess due to the risk of injuring other students.
If a medical note states “until further notice,” or restricts your child for more than 7 days a follow-up clearance note is required. This applies to the end of the school year as well. If your child is medically excused on the last day of school, a return-to-play note will be required for September.
If your child requires assistive devices due to an injury (such as crutches or a CAM boot), a medical note must be provided with specific instructions (e.g., non-weight bearing, need for a book buddy, early class dismissal, etc.). If your child will need a wheelchair in school, please contact the school nurse for procedures.
