Welcome to the High School West Health Office 

 

The School Health Office supports the health and safety of our students throughout the school day. Our nurses provide care for illness or injury, assist with managing medical conditions, and help ensure that all required health records and immunizations are current. We work closely with families and staff to promote a safe and healthy learning environment.

Attendance:

If your child will be absent from school please contact the attendance office at 631-592-3207

 

When to stay home:

If your child has a fever, please keep them home until they are fever free, WITHOUT MEDICATION for 24 hours.

If vomiting/ diarrhea, keep home until symptoms have resolved for 24 hours. 

If child has strep throat or pink eye please keep home for first 24 hours of treatment.

 

These policies help us to protect fellow students and staff. We appreciate your cooperation in this matter.

 

Emergency health contacts:

 Please make sure all of your information on Infinite Campus is correct. In the event of illness or emergency we need to be able to easily contact you. If you need to change any information that is listed please email our MIS department with the changes. MIS@hhh.k12.ny.us     

Please give them your name, student name and ID number along with any changes necessary.

 

Mandated Physicals

 

9th Grade is a mandatory year for all students to have a completed physical submitted by their doctor on the first day of school dated within one year of the first day of school.

 

11th Grade is a mandatory year for all students to have a completed physical submitted by their doctor on the first day of school dated withing one year of the first day of school.

It is also mandated by NYS to have a Meningococcal Vaccination at the age of 16 years old. If your child is 16 at the time of their annual physical please have this vaccination completed. It is mandatory to enter  12th grade on the fist day of school once 16 years old.

 

Per NYS DOH, children without the mandated vaccines are subject to exclusion from school until the requirement is met. 

 

Medication in school:

If your child is required to have a medication of any kind during the school day, it is required that we have a doctor's order and permission from the parent. All medication must be in a pharmacy labeled bottle/ package. All medication and the associated orders need to be dropped off at school by an adult unless the child is confirmed as a self carry for this medicaiton. ( This only applies to the emergency meds proviced) 

Please do not send your child to school with any medications that do not have doctors orders.

Emergency medications such as Epi-pens and asthma inhalers all require orders from the dr. These are the ONLY orders that a student may carry with them in school. 

Please note medication needs to be picked up at the end of the school year by an adult. Any medications left on the last day of school will be discarded. We can not keep any medications in the building over the summer. 

*** New medication orders and medication are due at the start of each school year at all levels. 

 

Physical Education / Sports Excusal:

Your child may be excused from PE / Sports ONLY  with a note from a doctor. If the note does not have an end date then we will need another note form the doctor clearing your child to return to PE / Sports. This includes if your child is not cleared by the last day of school or has an injury over the summer. We will then require a note on the first day of school clearing them to participate. 

 

If your child needs any accommodations such as: crutches, boot, 5 min pass, book buddy, elevator, or scribe, this will need to be written in the doctors note.

 

Concussion Protocol: 

HHH protocol states that if your child has a suspected or confirmed concussion they will need to be seen by a specialist. To fully return your child must complete Return to Play Protocol with our athletic trainer Zack Restucci    Zrestucci@hhh.k12.ny.us   631-592-3285. If your specialist is not in network with ThinkSmart your child will need to cleared as well with our district concussion specialist.  

 

Interscholastic Sports:

If your child wants to participate in after school sports, we need a current physical on file ( within the past 12 months from the start date of the sport). You must register on FINAL FORMS. The portal for each sport opens 30 days prior to the start of the season. Please select your sport FIRST. and then complete and sign all student forms and parent forms. Students must provide a "private email" in order to access the forms to sign. School emails  DO NOT WORK on final forms. 

 

Please keep in mind if your child requires any rescue  medication in school, or has a past diagnosis for a food allergy or asthma , we will need to have medication orders on file in order to cleared your child to play. If your child no longer needs this mediciaon we will need a note from the doctor stating this as well.
