New York State Health Requirements

 

Mandated NYS Physical Examinations
New York State requires a physical examination on file for all incoming 9th and 11th grade students. Physicals must be dated within one year of the start of school.

 

Students Entering 12th Grade
New York State requires all students entering 12th grade to have a record of the Meningococcal Conjugate Vaccine (Menactra, MCV4, MenACWY, or Menveo) on file with the Health Office. The vaccine must be administered on or after the student’s 16th birthday.

 

Students Entering 11th Grade
New York State requires students entering 11th grade to receive school screenings for near vision, distance vision, and hearing if documentation of these screenings is not included on a physical examination completed within one year prior to entry.

 

Students Entering 9th Grade
New York State requires boys entering 9th grade to receive a scoliosis screening if documentation of the screening is not included on a physical examination completed within one year prior to entry.

 

Please submit all updated physical examinations and immunization records to the Health Office as soon as possible. Students who are not in compliance with New York State immunization requirements may be excluded from school.
