Guidance Counselor Questionnaire

• This form should be completed and submitted to the guidance counselor by October 1 of the Senior Year.

Teacher Recommendation Information Profile

• This form should be provided to teachers who have agreed to write a letter of recommendation on behalf of the student.

• This is a fillable form that students can personalize, download and save to to their computer.

Parent Information Sheet

• This optional form should be completed by the parent/guardian and returned to the guidance counselor by October 1 of the Senior Year.

FERPA Waiver

• This is a sample only. This form must be completed with the student's Guidance Counselor.
