Attendance
If your child will be absent from school, please call Attendance at 631-592-3348
When to Stay Home
If your child has a fever, please keep them home until they are fever free, WITHOUT MEDICATION for 24 hours. This policy helps us to protect fellow students and staff. We appreciate your cooperation in abiding by this rule.
Vomiting within 24 hours
Diarrhea
First 24 hours of antibiotic treatment for strep throat or pink eye
PLEASE keep your child home if they aren’t feeling well.
Emergency Health Contacts
Please make sure all of your information on Infinite Campus is correct. In the event of illness or emergency we need to be able to easily contact you. If you need to change anything, please email our MIS department with the changes. MIS@hhh.k12.ny.us Please give them your name, student name and ID number, along with the changes to be made.
Mandated Physical/Physical forms
Per NYS all 7th grade students are required to have a 7th grade physical on file with the school. If a full vision screening and/or hearing screening are not completed, your child will receive the needed screenings in school by an RN.
7th grade girls will receive a scoliosis screening if not completed on their physical.
The only form we can accept for a physical is the Required NYS School Health Examination Form. Please find a link below to print a copy for your physician.

Mandated Immunizations
6th grade
Tdap vaccine is required for all students in 6th grade when they turn 11 years old. Per NYS Department of Health, the vaccination needs to be completed within 2 weeks of the 11th birthday. If your child is entering 6th grade and is age 11, please be sure the vaccination is completed prior to the first day of school. All documentation needs to be sent to the Health office.
7th grade
Meningococcal vaccination is due BY THE FIRST DAY OF 7TH GRADE. It is not based on age. This is GRADE dependent.
Per NYS DOH, children without the mandated vaccines are subject to exclusion from school until the requirement is met.
Medication in school
If your child is required to have a medication of any kind in school, it is required that we have a doctor’s order and permission from a parent. All medication must be in a pharmacy labeled bottle/package. All medication and the associated orders need to be dropped off at school by an adult unless the child is confirmed ‘self carry’ by the physician (applies to emergency medications only).
Please do not send your child to school with any medications (over-the-counter and prescription) that do not have doctor’s orders.
Emergency medications such as Benadryl, Epi-pens and asthma inhalers all require orders from the doctor. These are the ONLY medications a student may carry with them in school.
Please note, medication needs to be picked up at the end of the school year by an adult. Any medications left on the last day of school will be discarded. We cannot keep medication over the summer.
*New medication orders are needed for each school year, at all levels.
Physical Education Excusal/Sports Hold
Your child may be excused from PE class for up to one week of PE with a written note from a parent. Longer time periods require medical notes. If your child has a medical note that states “until further notice” we require a follow up note for a return date of play. This includes for the end of the school year as well. If your child is medically excused on the last day of school, we require a return to play letter for September.
If your child needs assistive devices while injured such as crutches or a CAM boot, we need a medical note stating the need with explicit directions such as ‘non-weight bearing’, book buddy, early class dismissal, etc. Please keep us informed of all follow up visits with the medical provider.
*Please note, if your child is excused from PE class, they may not participate in recess and competitive sports.
Concussion Protocol
HHH has a certain protocol to follow for all concussions, per Dr. Mark Harary, the district concussion specialist. If your child has a suspected or confirmed concussion, regardless of where the injury occurred, please contact the Health Office for guidance.
Interscholastic Sports
If your child (current/entering 7th and 8th grade only) wants to participate in an afterschool sport, we need to have current physical on file (within the last 12 months of the start of the sport). You must register your child on FINAL FORMS, found on the district website under ‘district’ then scroll to ‘athletics’. The portal opens 30 days prior to the start of the season. Please choose one sport only. A parent needs to complete the parent sections and the student needs to complete their own section. Your child needs a ‘private email’ in order to access the forms to sign. School emails DO NOT WORK on Final Forms.
Please keep in mind, if your child has rescue medications in school, or a past or current diagnosis for a food allergy or asthma, we will need to have medication orders on file in order to clear your child to play.
