ATHLETIC MEDICAL CLEARANCE
All students interested in participating in a fall interscholastic athletic sport must receive medical clearance prior to tryouts. Before you make an appointment with any physician for an examination, you should speak with the school district nurse about regulations and guidelines that must be followed:

Only the Half Hollow Hills yellow Pre-Participation Form and Physical Examination Form will be used for medical clearance. All forms must be picked up from the school nurse, completed and returned to the school nurse. Forms from private physicians will not be accepted. No faxed or attached physical forms will be accepted.
Remember, all yellow athletic forms must have a district physician's signature on it before your child can play a sport. The nurse will have a district physician review and co-sign the physical done by your private physician. The dates the district doctor will co-sign the forms are 8/16/11 and 8/22/11 so be sure to turn them into the nurse prior to these dates. Once they are signed by the district physician, the nurse will give the white card to the coach. Any questions, please call the nurse. Once these dates have passed, it will be your responsibility to have the yellow form reviewed and co-signed by the district physician before handing them into the nurse.
All forms must be returned directly to the nurse. Be advised that no one except the school nurse will assume responsibility for these forms. Please return these forms directly to the nurse during the hours listed above.
The school physician will be doing a limited number of free athletic physicals. Please contact the nurse directly to set up an appointment.
Remember
1. Only completed original yellow forms are accepted with no attachments.
2. Yellow form must be signed by a school physician
3. Return all forms to the nurse during the hours listed above.
